What type of insurance do you accept?

San Luis Transitional Care’s medical and rehabilitation services are covered by some managed healthcare organizations, group medical and worker’s compensation plans with prior authorization. We are Medicare-certified and our working knowledge of the ever-changing regulations is extensive. Please contact our Billing Office directly for more specific questions about your insurance provider.

What is Medicare?

Medicare is a health insurance program for people 65 or older or people under 65 with certain disabilities or end-stage renal disease. For patients meeting requirements, Medicare helps cover the costs of your stay. Our admission team members will be happy to provide you with current rates and coverage.

What are the different levels of care available?

Many patients need additional medical and rehabilitative therapies before successfully transitioning from hospital to home.  San Luis Transitional Care specializes in providing shorter-term rehabilitative care, with the focus on discharging back to your residence. However, if you require more extended care, our discharge planner works directly with you to help facilitate a transfer to the level of care that best meets your longer-term needs.

What should I look for when choosing a skilled nursing and rehabilitation center?

For short-term stays focused on rehabilitating a patient home, ask the center about its successful outcomes treating patients with your or your loved one’s illness, injury or disease. Ask about the staff’s knowledge about post-hospital care. Tour the center for cleanliness, friendliness of staff and amenities that are important to you.

Can I tour the facility before my family member comes to the hospital? Do I have to make an appointment?
