Time passes quickly as therapy visits alternate with our recreational program, quiet relaxation and delicious meals served in your room or in our gracious dining area. Guest meal service is provided with advance notice should your loved ones wish to join you.

Each room is beautifully appointed to include a color television, personal telephone complete with your own extension, call system, tiled bathroom, fine bed linens and a private, landscaped patio.

Our Admissions Coordinator expedites insurance authorization and coordinates the admission process working as a tandem with the acute care Case Managers and Physicians to ensure a smooth transition.

San Luis Transitional Care accepts Medicare, some managed care health plans and private insurance.
